This procedure usually involves injections of a dense hyaluronic acid-based filler such as Juvederm or Restylane. Hyaluronic acid is a naturally occurring substance in the body and serves as a lubricating and cushioning agent in the joints. Hyaluronic acid possesses hydrophilic (water-attracting) properties and expands in volume when injected into the lips. 

First, the lips are sanitized with alcohol and numbed using a topical lidocaine cream, which stays on for 15 – 20 minutes.

Once the lips have been carefully marked for injection, a hyaluronic acid-based filler is injected vertically into various regions of the lip, using a tiny hollow needle (20 mm, 31 Gauge), through entry points along the vermilion border including Cupid’s bow, which is the primary focus in the center of the upper lip; body of the lip; and oral commissures (corners of the mouth).

Just like most other lip fillers, the Russian lip style utilizes hyaluronic acid dermal fillers to enhance the volume and structural definition of the lips. However, there are some major differences in the injection technique itself as well as the results, when comparing this innovative lip augmentation method to the standard lip filler.

Russian lip Injection:

While traditional lip fillers are injected horizontally, often resulting in a “sausage effect”, the Russian lip injection is performed vertically, to uniformly increase the size and the vertical dimension of the lip from top to bottom instead of right to left.

Because this method requires more entry points than the standard lip filler technique, the procedure itself takes slightly longer (30 – 45 minutes), while a regular lip filler takes 15 – 30 minutes.

The filler is placed into the superficial subdermal plane, only 1 – 2 mm deep to effectively stretch the overlying skin. Additionally, the superficial subdermal plane is relatively avascular, meaning that it doesn’t have too many blood vessels, such as superior and inferior labial arteries and veins that can be inadvertently punctured and lead to bruising, hematomas and skin discoloration.

Russian lip unique results:

While traditional lip augmentation results are often wider, the Russian lip results in a taller, coveted heart-shaped lip creating the Russian doll lip effect. There’s more focus on the central part and body of the lip, accentuating the cupid’s bow and drawing the eye to the mid-lip, rather than the outer corners of the lip, which can otherwise result in a “trout pout”.

The bullhorn lip lift, also known as a subnasal lift, is a surgical procedure used to enhance the upper lip by rolling it out and upward by shortening the philtrum, the region between the nostrils and top lip. The operation involves a small incision made beneath the nostrils, allowing space for the upper lip to be lifted in a vertical manner after which the incision is sutured. This method is effective in creating an enhanced upper lip permanently, though its surgical nature requires up to 2 weeks of downtime, including prolonged bruising, substantial swelling, and painful discomfort, particularly while making facial expressions, eating, or even sneezing. A common side effect associated with the bullhorn lip lift is scarring of the lips along the incision, which often requires additional treatment with microneedling or laser resurfacing.

Lipotransfer is a procedure in which fat is harvested from one part of the body, typically the abdominal region, and transferred to the lips via injection after the collected adipose tissue has been purified. While this treatment does involve lip injections, the surgical aspect is that of the liposuction required to harvest the fat, which involves general anesthesia and up to 2 weeks of post-op downtime. Risks and side effects may include infection and lumpy lips, as the metabolic process of purified fat in the body is unpredictable, meaning that your lipotransfer may degrade unevenly over time.

Surgical lip enhancements are great for patients looking for long-term results, though they do come with an array of potential complications that may be irreversible. The Russian lip technique is administered using a hyaluronic acid based dermal filler, which is metabolized by the body over time. While hyaluronic acid fillers are not permanent, the Russian lip technique comes with essentially no serious risks or side effects and minimal to no downtime. The Russian Lip procedure is comparable to that of the average lip filler, though the injection process and results are slightly different.

Right before the injections are started, a makeup remover is used to wipe off any makeup or residue, followed by sterilization of the treatment area using alcohol. A topical numbing cream containing benzocaine, lidocaine, or tetracaine is applied to your lips 15 – 20 minutes prior to injection to prevent unnecessary discomfort. Once numbed and sanitized, the lips are injected with a hyaluronic acid filler vertically, either through a small hollow needle or a blunt cannula utilizing a retrograde serial puncture technique and gentle artistry in which the lip is molded vertically to create a natural-looking, heart shape.

Once the injections are complete, the treatment area is once again sanitized and you will be given an ice pack to reduce swelling and bruising. You will then be able to go home immediately. Rubbing or touching your lips is contraindicated to avoid migration, and exercise should be avoided to reduce the chances of excessive swelling and bruising. If there are any signs of hematomas, you may apply a warm compress on day 2. 

The results of Russian doll injections are instantaneous and the lips will immediately become augmented and contoured, though the final results and shape will not be obtained until the swelling and bruising have subsided, within 1-4 days post Russian lip treatment.


If performed by an experienced and skilled board-certified plastic surgeon, complications are rare. However, every minimally invasive medical procedures carry the possibility of risks and side effects, including transient swelling and ecchymosis (1-4 days), minor bleeding at the injection site, skin irritation, and discoloration. A rare complication is a cold sore reactivation secondary to stress and anxiety, which can be immediately treated with a short course of Valtrex.

During the first few days your lips may look asymmetric and disproportionately large due to swelling and/or bruising. Keep in mind that this is part of the process and will resolve once the swelling and bruising have gone down.

Very rarely more serious complications may arise either through poor sanitary processes or not following post-care instructions. Bacterial lip infection is usually secondary to an inadequate sterile environment and can be completely prevented if standard surgical protocols are in place. 

Some fillers, such as Restylane Kysse provoke an intense inflammatory response leading to the formation of small nodules, knots and lumps, which represent a thin fibrous capsule around filler particles. Dr. Schwarzburg uses microscopic Hylenex injections to dissolve those tiny granulomas without any sequela. 

Skin necrosis or tissue death or inadvertent vascular compromise are extremely rare, but devastating complications resulting from inadequate training and lack of basic anatomical knowledge. 

Initial visual asymmetry should not be a reason for concern as sometimes foreign body reaction can be more intense in some parts of the lips than others. Filler migration, where the gel particles move through the lip tissues alongside poor placement technique or not following the post care instructions. 

The difference between an experienced board certified physician in a city like New York and an inexperienced or unqualified injector is their ability to safely identify and handle these risks. There is no significant medical or social downtime associated with Russian lip filler injections. Minor side effects do occur with any medical treatment involving needles. To expedite the recovery you may apply cold therapy such as an ice pack right after your Russian lip filler treatment and throughout the first 24 hours. The purpose of the ice pack is to constrict the blood vessels, which reduces blood flow to the recently injected lips. Reduction of blood flow reduces production of inflammatory signals and leads to less swelling and pain. This same principle lies behind avoiding hot environments such as saunas and steamy showers, as they promote unnecessary blood flow to the traumatized lip area. Hence, avoiding heat for 48 hours post injection will also decrease swelling.

There is anecdotal evidence that applying aloe vera and arnica cream helps with minor bruises.

Dr. Schwarzburg, who specializes in cosmetic dermatology and is located in NYC, also recommends staying away from touching, massaging or puckering your newly created Russian lips as this can lead to potential filler migration. Filler displacement is very rare and can be completely prevented by following the precautionary steps above. 

It is also advised to wait 30 minutes before you apply makeup so that the little needle wounds completely seal to ensure that there is no bacterial contamination.

Studies have shown that increasing water intake and limiting sodium and alcohol tend to promote better healing and minimize the degree of swelling.
